Respite includes supervising or providing supports for individuals with daily living activities such as personal care, while caregivers get a temporary break. Our planners can assist your family with applying for funding and help you navigate the services available to you.

If you are choosing to manage your own funding, the Agency can assist you with navigating and using respiteservices.com to select your own in-home worker. Alternatively, if you choose to purchases services directly from us, we will provide the fully-trained and qualified staff to provide support, in keeping with your plan.

I have received Passport funding and want to start developing a person-directed plan. Where do I start?

If you have or are seeking Passport, SSAH or other funds to purchase or arrange for services, Community Living Wallaceburg has you covered. How you proceed is your choice.

SELF-MANAGED SUPPORTS 

Choosing to manage your individualized funding, the Agency can assist you with the following:

  • Navigation and use of respiteservices.com
  • Hiring and training your own in-home worker
  • Accessing a wide range of community services
  • Transition planning from school to work, community and housing
  • Creating a self-directed plan
  • Administration of Passport, SSAH and other funding
  • Developing service agreements

PURCHASE OF SERVICES

Alternatively, you may choose to purchase services directly from Community Living Wallaceburg and benefit by:

  • Not having to hire staff or manage payroll
  • Having fully trained and qualified staff provide support in keep with your plan
  • Scheduled support with on-call back-up; services monitored and operated within Ministry regulations, Quality Assurance Measures, and safety standards
  • All records, reports, and financial responsibility managed by the agency.

Community Living Wallaceburg is committed to providing quality services to families. We can assist with navigating the funding process by guiding you through the application steps.

Developmental Services Ontario, South West Region (DSO-SWR) is the single point of access to developmental services funded by the Ministry of Community and Social Services. DSO determines eligibility for services and funding and assists with completing applications by identifying service and support needs.
