The first step would be to connect you with one of our Outcome Facilitators who will work with you to identify what you want to achieve and what steps are required to reach these goals!

Outcomes Facilitators at Community Living Wallaceburg are flexible, creative and collaborative and guided by values and principles who focus on individuals and building their future. They expertly and confidently guide people currently receiving or requesting services from or through our Agency to imagine and articulate a vision for their future, to act on the steps to achieve their vision and to reflect on the impact of changes to their lives.

The PATH (Planning Alternative Tomorrows with Hope) captures a person’s dream in a visual representation to help them to identify their dreams and goals, control the planning process and move towards achieving their dreams and goals. Whether it’s finding an apartment, getting a job or new volunteer opportunity, reconnecting with an old friend or relative, or even getting married and having a family, no dream is too big!

Once you have thought about some of those important questions, we can help you design your own plan of action.

The Outcome Facilitator will be available to you to help you along your path together with a dedicated support team.

Your support team will be able to help you succeed at the items you want to do.
